Wiki BRACA Test in office Please help asap

The Dr I work for wants to start offering BRACA testing in our office. We would do the blood draw and send to lab, then counsel pt when results are in. Would I use an 81214 and 81216 or just the 81211. Also do I need report the 36410 for the draw also? Thank you
 
you do not bill the lab codes unless you perform the lab work in the office , or you do the billing for the lab. If you draw the blood and send the specimen out and the lab does their own billing you get to bill the blood collection fee only
