"Metastatic poorly differentiated adenocarcinoma of the breast on Letrozole s/p bilateral mastectomy in 2009."

Would this dx of breast cancer be considered active because she is still on medication? Or it can't be active because of the mastectomy?

Any input would be great!

A maintenance drug to prevent recurrance is not considered active treatment. This would be a history-V10.3
 
if it is stated as metastatic, there should be a metastatic site, if the site is unknown you can use 199.1 and the V10.3 for the scenario.

A drug given to prevent recurrence of the cancer is still treating the neoplasm and it is still coded as active. If it is metastatic breast cancer there should be a metastatic site and if none is documented you use the breast cancer code for the primary site with 199.1 for unknown site for the secondary site.
