Wiki broken epidural catheter

My surgeon was called in to find a broken epidural catheter. His OP note procedures are:
1. Attempted foreign body removal from the back
2. Intermediate layered closure of 2.5 cm back wound
3. Intraoperative soft tissue ultrasound to the back

I cannot find a code for 1. and have found that there isn't a code for wound exploration for back/flank. Also the catheter was lost in the muscle and never found. :eek:

Can anyone point me in the right direction?
