Wiki Bronchoscope and Laryngoscope

When the above two scopes are performed, how do you know when one is inclusive to one another or when to add mod 59? When reading OP notes, i look to see if they looked at two different areas for possible different reasons. Doesnt the bronchoscope sometimes pass over the same anatomy as the laryngoscope.
 
A bronchoscope passes over the same anatomy but reaches further into the trachea and/or lungs. So why did they use the laryngoscope too? Checking anatomy? Better visualization of cords? If this was a difficult intubation they may have needed the laryngoscope to insert the bronchoscope but there needs to be a reason they used both.
What Dx code(s) are you using?
 
Thank you for your response, this particular case is J38.6 stenosis of larynx,,R05 cough, J98.4 other disorders of lung. I am in a peds ENT specialty. I dont get any edits with these two services either.
