A E&M occurs that results in the decision to perform surgery on the same date. A diagnostic bronchoscopy is performed and then a thoracotomy at the same session. I have a consultant that is advising to add both the modifiers 57 and 25 to the E&M code--the modifier 25 because the bronchoscopy was performed and the modifier 57 because a thoracotomy was performed. What is your opinion?
 
You only need the -57 modifier

If you performed a major procedure (90 day global) then you only need the -57 modifier, even if you also performed a minor procedure (0-10 day global).
