Hi, I'm a biller and am trying to understand how to correctly submit for a Spravato visit after using Buy & Bill to procure the medication. Please correct me if I'm wrong, I believe 99215(1 unit), 99417(4 units) and S0013 (56 mg = 56 units) is the equivalent of G2082. How do I determine how to submit the claim (bundled vs non-bundled)? It's a commercial insurance plan with a behavioral carve out. Are there more factors I need to be aware of? If I am to submit using 99215/99417/S0013 - should I submit them on two different claim forms? Any and all input would be appreciated. Thanks in advance.
