Is CPT 20936, autograft for spine surgery, bundled with CPT 22851, arthrodesis? or can I report them separately?

These are both add on codes, and cannot be separated from the primary CPT intended for each.... What is the primary code? If you are unsure, take a look in your CPT book. The primary CPTs for each should be listed.

The primary procedure is CPT 63030, Laminotomy (hemilaminectomy), with decompression of nerve root(s).

Per my software, "Procedure 20936 is an ADD ON code and should never be billed without code(s) 0195T-0195T - 0196T-0196T - 22319-22319 - 22532-22533 - 22548-22551 - 22554-22558 - 22590-22612 - 22630-22630 - 22800-22812." So, apparently, you cannot combine 20936 with 63030 & 22581. Was there something additional that you're trying to bill for? If so, for what services?

My software states, CPT Add-on code, use 20936 in conjunction with 22319, 22532, 22533, 22548-22558, 22590-22612, 22630, 22800-22812.

Please post the OR, 20936 and 22851 are codes usually used with fusion codes, 22612,22630, 22633, 22634.
