Please help. Here is what surgeon performed for ruptured AAA:
1. Endoluminal graft repair of ruptured AAA w/aorto-uni-iliac stent graft
2. Placement of left iliac extension limb
3. Placement of Amplatz plug, right limb of the iliac graft
4. Bilateral common femoral endarterectomy
5. Femoral-femoral bypass
6. Intravascular ultrasound exam of the aorta and bilat common iliac arteries
7. Bilat femoral artery cutdown with exposure of the common femoral arteries
8. Aortoiliac angiography
My question is: Are both 34825 and 34812 included with 34805? They were bundled by insurance and it looks like they may be components of 34825. I am not a coder so any assistance you can give would be greatly appreciated. Thank you!
