Hi,

Patient comes to ER for removal of cast which was applied 2 days before in another hospital, the patient was not able to tolerate the pain with the cast so he wants to remove it. The patient has distall radius fracture this fracture happened 2 days ago and for the current visit is it appropriate to code V54.12 followed by 813.42 or we should code only 813.42..(because the active fracture has been treated by the physician two days ago by applying the cast)
 
Since the dx code is the patient's diagnosis, and this patient has already had the initial treatment for the fracture 2 days ago, you code only the V54.12, you never code the V54.xx code with the code for the acute fracture unless the patient has sustained a new acute fracture while being treated for a different healing fracture.
