They way I have read the NCCI edits is that 28110 is in with col 1 and nothing is listed in col 2 for 28308, however in reverse col 1 has 28308 with 28110 in col 2 but has a 1 in the modifier col. So I can add the 59. The op report says it is a seperate proc. and the cpt book lists 28110 as seperate proc. Help ? Doc billed for both professionally and was paid for both. We want to bill for both from the ASC.
