Is there a CPT and ICD 9 code if the patient is called to exam room and written up by nurse, then leaves before the Dr. comes in?
 
Since you are helping me, i have another question. What if a patient comes in because they need a wic form filled out and nurse had to get height and weight. Didn't see a dr.

I'm not 100% sure on this one - I'd say either V70.3 (Other exam for administrative purposes - eg, insurance certification) , V68.89 (Other specified administrative purposes), or, if a certificate was issued, V68.09 (Other issue of medical certificates.

Height and weight are technically exam elements, but I don't know if it's enough to justify the V70.3 or not. You should check with your state Medicaid office on how they want that billed. You'll probably have to add a 32 modifier on whichever CPT code you use.
