Wiki can/lead issue after generator change

How would you code :

"The RV lead tip was found to be fully inserted but easily removed from header with gentle traction. Lead tested with stable parameters. Reinserted and secured with resolution of RV noise. Atrial and LV leads found to be firmly connected. A Tyrx pouch was placed".

initial ICD implant was 2015.
came in for a generator change, found the RV was not secure in the header, but all leads were secure in body position.
