She was given a Yankauer suction as well as a sweetheart tongue retractor to assist in the procedure. Hurricane spray was sprayed onto the soft palate as well as the nasopharyngeal and oropharyngeal wall. 3 cc of Afrin and tetracaine mixture was sprayed into bilateral nares with an atomizer. The nasopharyngoscope was placed through the right nasal cavity to visualize the velopharyngeal port. There was noted to be a circular gap present on the right side of the velopharynx. The coincides with the scarring on the soft palate. A fresh syringe of Juvederm Voluma XC was prepared with a 27-gauge 1 1/4 inch needle. The syringe was placed through the mouth and the needle passed into the palate to inject into the posterior aspect of the soft palate under endoscopic guidance. The site of injection was extended in multiple areas on the right side of the soft palate and also on the right right side of the nasopharyngeal wall. Upon completion, there were only a trace amount of leakage or bubbling noted with plosive formation. The scope was then removed the nose. She tolerated this well without any complication. She did have noticeable improvement in the resonance of his speech upon completion.
