ok so ive never run into this and i need your help

DX 1. Attenuated midline abdominal fascia w/o obvious incisional hernia
DX 2. Umbilical Hernia 553.1

Procedures performed 1: Excision attenuated midline abdominal fascia and repair of fascial defect w/ mesh

2: umbilical hernia repair 49585

please help

i would probably use cpt 50728 and dx 756.70 unspecified, but of course, it must depend on what is documented specifically. cpt 50728 includes hernia repair too.
