Wiki Can we consider established conditions as chronic in pain management E/M coding

If a patient has conditions like lumbar radiculopthy, cervical spondolysis, lumbar facet arthopathy... For less than a month and again coming 2nd time to office visit for same conditions follow up. So can we consider it as 2 or more chronic illness to take MDM risk table as moderate?

Can anyone suggest me on this type of scenario....

In most cases the term chronic condition would involved a time span of 3 or more months (US Department for National Health Stats). The only exception to this would be the physicians documentation that the patient does indeed have a "chronic condition" as defined by previous medical records or history obtained.
