Can you bill for an extensive laminectomy/smith peterson osteotomy with decompression for severe stenosis at L3-4 along with TLIF at the same level and extending posterior segmental instrumentation from T12-L5? Per the surgeon, this far exceeded the laminectomy/discectomy to prepare the fusion bed. Please advise. Procedure details include:
1. Extensive laminectomy L3-4 with decompression of severe stenosis
2 .Left sided hemilaminotomy and partial facetectomy L1-2, L2-3 for stenosis
3.Total diskectomy L3-4 for nerve root decompression, preparation of fusion bed and reduction of spondylolisthesis.
4. Left-sided transformaninal lumbar interbody fusion with PEEK cage L3-4
5. Posterior segmental fusion T12-L5
6. Posterior Segmental fixation T12-L5 with depuy expedium screw and rod construct
7. Harvesting of autologus decompression bone
8. Harvesting of bilateral iliac crest bone marrow
