Occasionally our hospital discharges a patient and the ambulance attempts to take them back to a SNF. However, due to inclement weather conditions (icy roads), the ambulance has to turn around and bring the patient back to the hospital as it is unsafe driving conditions. My question is....can we just edit or void the original discharge order? We would be doing a direct admit so the patient wouldn't have to go thru the ER. If we can edit the discharge order, can you please provide a reference we can use for our new policy? Thank you for any help you can provide.
