Good morning. Can you report an intermediate closure with a punch biopsy? Our provider performed a punch biopsy. There was a significant hole that remained. He performed an intermediate closure on this wound. I have read per CPT guidelines simple closure is included in the service. But I do not see any information on intermediate and complex where punch is concerned. Thank you for any advice. :)
