Wiki can you use modifier 25 twice on same encounter?

Ok here is the dilemma; patient seen for a prevent and a problem visit, patient also receives immunizations. Would you put a 25 modifier on both the prevent & problem visit?

Do you know of a reputable site that will answer this in black-n-white?

Just put in on the preventive code only since the additional service would be the E&M

yes but so are the vaccinations, I heard both ways

I don't code primary care, but I checked with another coder and she said the 25 goes on the E&M not the Preventative.

you need to read the 25 modifier description which is- significant, separately identifiable evaluation and management service. A preventive visit is not an E/M service. The 25 goes on the problem e/m.
