Wiki Canceled Spinal Surgery

Hi,
The patient came in for spinal surgery, but it was cancelled right after anesthesia was started. Which of the following primary codes would be appropriate to bill with modifier 74? 22852 or 22633.

Are you billing for the surgeon or the facility?
I would also have questions about why a 22852 would be reported with 22633. Normally, if the plan was removing old and adding new at the same plus additional levels you can't report the removal. So, I would probably use the main code that would have been reported should the case have gone through.
