Does anyone know what is required to be in each report and where I can find this in writing?

For example...are all the following categories required:
1. Patient information, date & time of procedure
2. Procedures performed
3. Operators performing the procedure
4. Techniques & equipment used
5. Findings of the procedure
6. Complications
7. Final Impression & recommendations
 
The ACC did an article on structured reporting, I haven't read it yet, but it might help you. Also, look at your Medicare LCD's.
