I am being told that all services performed (cath, stent, angiography, etc.) need to be documented within the "body" of the report or it is "not billable" by a physician. My Drs. use a template that breaks the procedures down individually, capturing all procedures performed, but not necessarily within the "body". Does anyone have any further information regarding this?
