When a patient is seen in the hospital and Cardiac Clearance is requested, how do you figure out the level for the HPI, ROS and Medical Decision Making? Also, where could I find this information? I dont see anything in my notes that would explain this. Thanks!!!!!
 
Not sure what you're asking, are you looking at the notes for the provider who is requesting the cardiac clearance or the provider who is clearing the patient? Either way you will determine the levels based on what the provider documented in their respective notes.
