Good Morning!

We have a patient that just completed 34 of her 36 Cardiac Rehab visits from a stent placement and last week, she ended up with another stent. She will be starting a "new" course of 36 visits of Cardiac Rehab on Monday. Will CMS know this is another "episode" requireing Rehab or will we need to add the KX modifier on all of her next visits?
