Can someone please set the record straight regarding down coding and/or billing an ecat to a holter. Is it ever appropriate to down code the ecat and bill a holter? The ecat was used but due to unusual circumstances, a holter was billed; seeing this is technically a lower reimbursement, is it appropriate to bill for the holter?
