I work in outpatient coding......

When I am coding an EKG, and it is abnormal,
would i code Abnormal EKG AND the symptoms second?

Ive been told a few different things and would like to get clear answer!

Cardio Advise Needed

Code the findings first. Why or what was the Diagnosis of the abnormal findings? A-fib 427.31? Bradycardia 427.89?
 
If the EKG is normal, you code the reason the doctor had the test done, chest pain etc...
If results are abnormal, MI, and so on.
