I don't even know where to began with this procedure. He documented he did the following

Left Ventriculogram
Coronary angiography
graft angiography
PCI to the distal LAD and stent resenosis
PCI to the mid SVG to the diagonal graft body


The only thing i can get and not sure if it's correct is the following:

93458
93563 maybe

Geez, please help me!
