AMA's CPT Reference Guide for Cardiovascular Coding includes cardiothoracic coding and Dr. Z's has one as well, both are really good.

I have the Dr. Z but don't give me much tips for the open surgeries. And I found the AMA reference book. I'm in the process of buying it.
