I need documentation to show my physicians regarding the global definition of CPT 93015. They are employees of a clinic owned by a hospital system. They do not therefore own the equipment and need to use instead the 93016 and 92018, plus one of the 784xx series for nuclear stress tests. My difficulty is finding official guideline documentation regarding this to share with them. Any help will be greatly appreciated.
