If a physician is supervising and reading cardiovascular stress tests in an outpatient facility, do you bill both 93016 and 93018? We have been billing these and getting denied by medicare saying we need a modifier. We have tried with modifier 26 and still getting denied. Does anybody know which modifier they are looking for?
 
93015 is used if you own the equipment..

we bill 93016 and 93018 all the time here in AZ to Medicare and other commercial payors and are getting paid. They do not require a modifier. I would call Medicare directly, usually we have good luck and they will help you find the information you need to get the claim considered.

I have no idea what modifiers they are looking for, neither of those codes require a modifier.
