what exactly does the progress note state?

In the assessment it states " all of these conditions are stable and her carotids show mild to moderate diffuse bilateral disease with no intervention necessary at this point". Under the impression it states: Carotid disease, stable.
Some of the coders I work with want to use 447.9 or 447.8.
