If a patient has Left carotid endarterectomy with patch, and the clot is removed from the common, external, and internal with 1 patch closure would you only bill 1 time with 35301? That is how I normally bill it.

What if additional incisions were made into the internal carotid further up the artery with additional closure. I've tried looking everywhere to see if it would be appropriate to bill it 2x.

Any input would be appreciated.
