Hello, I haven't coded peripherals in a long time and vaguely remember learning that the provider needs to state in his note that it was a "selective" catheter placement. Here is his note. I was thinking this would just be 36222-50 but he was using 36223. TIA

The right innominate artery was engaged using 3 DRC diagnostic catheter and was subsequently exchanged over Versacore wire for a Navicross microcatheter and diagnostic of the right common carotid artery was performed with the Navicross microcatheter.

Digital subtraction angiography of the right anterior cervical circulation demonstrates a normal course and caliber of the right common carotid artery with bifurcation of the right common carotid artery at the level of C4. There is mild disease at the carotid bulb with normal course and caliber of the cervical right internal carotid artery.

Digital subtraction angiography of the right anterior intracranial circulation demonstrates normal course and caliber of the right internal carotid artery (petrous, lacerum, cavernous, clinoid, ophthalmic, and communicating) with bifurcation into the left anterior cerebral artery and middle cerebral artery. Normal venous phase without evidence of AV malformation.

The left common carotid artery was engaged with a 3 DRC catheter, which was exchanged for a Navicross microcatheter through which diagnostic angiography of the left common carotid artery was performed.

Digital subtraction angiography of the left anterior cervical circulation demonstrates normal course and caliber of the left common carotid artery with bifurcation at the level of C5. There is mild disease of the carotid bulb. There appears to be severe, calcified, subtotal (99%) disease of the ostium of the external carotid artery. The internal carotid artery appears to have mild disease at its ostium and proximal segment. The cervical left internal carotid artery has no significant disease.

Digital subtraction images of the left anterior intracranial circulation demonstrates normal course and caliber of the left internal carotid artery (petrous, lacerum, cavernous, clinoid, ophthalmic, communicating) with normal bifurcation of the left internal carotid artery into the left anterior cerebral artery and middle cerebral artery.
