Does anyone do case management billing and coding. I need to know what documentation to look for. It also looks like the provider will have to sign the note stating that he/she review. I have both behavioral health and family practice wanting to bill for this. Right now the documentation I am getting is just follow up calling and talking to others on the phone (family members. other providers) I have yet to see a plan in place for the patient . any information will be greatly appreciated
