Can A4590 be billed with Q4020?

The physician states that he put the patient in a long arm cast using 2" fiberglass material.

No, you cannot use both code, you can use one or the other depending on what the insurance company will accept. Both are for casting supplies, the Q codes describe the material and also whether the cast is for a child (10 and under) or and adult (over 10). There are also different Q codes for splints and casts. The code you have above is for a long arm splint, fiberglass, child 10 and under. The code for a long arm cast, fiberglass, child 10 and under is Q4008. These codes are more specific than A4590 which only states "special casting material (ie fiberglass).
