Does anyone have a copy of the form they provide to patients showing them the cost of the premium lens they are going to have to pay for prior to cataract surgery.
I would appreciate if you are willing to share or if you could tell me where I could locate some examples on line.

I do not have an example I can share, but it should clearly detail the type of lens and total out of pocket cost that goes to MD and ASC. It would also be beneficial for you to state this is non-billable to insurance and that there may be additional bills for any co-insurance, co-pays and deductibles that are in no relation with the upgraded service, but the cataract procedure itself.
