Wiki Category II codes - all Medicare patient's

I have a family practice that is using category II codes. They were told by their former billing agency to use them for all Medicare patient's. Below is an example of what was billed:

99213, 4037F (Influenza imm order/admin), 3014F (Screen mammo doc rev)

No other codes were used - if an Immunization was done should there not be two additional codes (one for the admin/one for the immu) also, the screening mammo?

Thoughts?

the F codes are used only if the provider agreed to participate is my understanding. They are non reimbursable and are submitted with a $0 or a $.01. Yes there needed to be the immunization admin and vaccine codes . The screening mam code only if he were the provider performing the mam, to go over the report is part of the E&M
