Wiki Category III codes and GA modifier to Medicare

Our pain clinic performed category III code 0232T injection of platelet rich plasma on a Medicare patient. I know Medicare will not pay for the service and that I need to add modifier GA, but what I don't know is if I associate a cost for the procedure or not???? Can anyone please help? We did have the patient sign an ABN.
