I see where 93458 replaced 2010 93510, however, with the 93510 modifier 26 was used, and all new cath codes for the most part, have been bundled with the imaging interpretation. Does modifier 26 need to be used with these bundled codes?

Yes, I would use the modifier-26 for the professional part of the charge.
