Can someone give me the guideliness for billing a left heart cath? I don't really understand as to what I need to look for in the documentation? Are there particular vessels that need to be accessed?

A true left heart cath will include coronary angiography (left, right) and crossing the aortic valve. You will need to see documentation of left ventriculogram or LVEDP (left ventricle end diastolic pressures).

You may see "LHC" performed but if only the coronary angiographies are performed, then you would not capture 93458-26 but rather 93454-26.

My physicians usually access the left or right femoral artery.
