Wiki Cath with PTCA

I need some help defining when it is ok to put a modifer 59 on the Cath and PTCA.
Example:

Pt came in thru ER. Cath was ordered. During Cath cardiologist decided a stent was needed. The Cardiologist proceded with stent during same session.

I know the Cath (93458) and Stent (92928) is bundled. But, a 59 is allowed.

Myself and fellow coders are debating if the 59 is allowed in this instance.

Thanks in advance for your advice.
