Wiki catheter placement for 93459

Hello,
I seem to be have some confusion on how to charge for 93459. Does the catheter need to go into the grafts to get credit for 93459? I am told that if the doctor dictates graft findings, a catheter had to be placed in the graft in order to be reported. Would this be a 93458 or 93459?

The dictation has to say that the graft was catheterized and findings of the graft to get 93459. 93458 is a LHC w/ coronaries.

Would have any documention or link that would support that the cath needs to go in the grafts other thatn the CPT book that I could show the doctors here at the clinic?
