Approach was right brachial up and over the aortic arch and selectively cathed the left axillary. Would that be 37215 or 37216?
 
Never mind..I figured it out ;)

Answer~Doesn't matter what approach (femoral or brachial) left extremities; subclavian and axillary are 1st order 36215.
