If a patient comes in for symptoms of a U.T.I. and we documented it in the C.C. but we didn't document it in the HPI. We documented it only in the ROS. In the HPI , we documented the other ongoing treated conditions. Shouldn't we have documented the UTI symptoms under the HPI and not in the ROS only?
