Wiki "CC" for new patient/consult

Is a "cc" needed for a newpt/consult OR just the reason for the consult/new pt visit. I just want to clarify that this is not just for f/u. I had someone ask from our training department that if a patient is being seen for their 6 mo follow up and they have no new complaints,can they just state "6 mo follow up"

Our MAC requires a little more detail...

"Indicate clearly the chief complaint and/or reason for the visit. Don't limit the chief complaint to "follow-up" without identifying the problem(s) being followed."
