Wiki CCM billing question

Hello, if a patient has 30 minutes of CCM time should we be billing 99490. I know you would use 99491 for just provider time and most of the encounter calls are with the nurse.

I was told that we can use the 99439 which is 40 minutes as an add on if it meets the increment
