When my provider documents/orders 1000 mg of Rocephin, he documents it like this: Ceftriaxone sodium injection Seq #1: Strength 1, Dose 1 g. To code this, I mentally convert the gram into mg and use HCPCS J0696 x 4 units. What I need to know is, does the dosage need to be documented somewhere in the visit record that 1000 mg was ordered, or is it okay for me to just convert the dosage and bill?

Dosage must be documented, but it doesn't have to reach 1000mg. It can read the 1 gram.
