We have a common encounter type that I would like to have opinions on to make sure we are coding these correctly, please.


We have many Powered Vehicle Operators (forklift drivers) that need annual certification. The only information that is needed from us for them to certify the employee is:

1) Collection of a random urine drug test

2) Height and weight (because if > 300 lbs., they will get additional body measurements to be used for obtaining a special harness they must wear)

3) A vision test



Essentially a nursing visit.


BP readings were also added along with also obtaining, or updating, complete PFSH on each patient. Abnormal BP results are consulted with the NP or MD who may see the patient as well (giving a 99212 code, if indicated).



Now regarding the coding, we charge:

Dx: V70.5

99211 Office Visit (per WHM)

99173 Visual Acuity

Dx: v70.4

80100 Drug Screen

99000 Handling of specimen from office to lab

This does not seem right to me. I think that we are using the wrong codes here especially when it comes to the vison and drug screenings.

Any ideas on correct coding for these visits:)? Thanks.
