Is it okay if the the CMA are the one documenting the HPI notes?

No, only the provider can document the HPI. Nurses can document the CC and the ROS, and the PFSH.

Hi Linda,
What if the CMA document in the CC and same notes in the HPI that this visit is for venipuncture can we bill for 36415 only but using the Provider's NPI?
