Please help. I am getting denials for E0855, they are looking for a modifier. I cannot find that one is required. Anyone know what they are looking for? thanks

Hi - Did you check for an LCD/LCA for the payer? For instance, https://www.cms.gov/medicare-coverage-database/view/article.aspx?articleId=52476 has a section on modifiers KX, GA, and GZ.
